Referring a young person for mentoring should feel straightforward.
Jack works collaboratively with schools, colleges and professionals to understand the young person’s circumstances and determine whether mentoring is an appropriate form of support.

HOW IT WORKS
From initial conversation to ongoing support
1. Initial discussion
Contact Jack to discuss the young person’s circumstances, current support and whether mentoring could be suitable.
2. Complete the referral form
Provide the relevant details, reasons for the referral, identified concerns and the outcomes you hope mentoring may support.
3. Referral review
Jack reviews the information and may contact you to clarify any details or discuss the proposed support.
4. Agreement and consent
If mentoring is appropriate, the support plan, practical arrangements, goals and necessary consent are agreed before sessions begin.
4. Mentoring and review
Jack builds a relationship with the young person, works towards agreed goals and provides appropriate progress updates and a final summary.
